Waiver of Liability Form
Each Participant is required to have a completed waiver of liability form in order to attend Basics of Faith Retreat. Both student AND adult participants must have a form. Please Click the button to download the PDF of the form and be sure each participant completes one. For your student participants, you (the group leader) MUST witness the parent / guardian signing the form. If you are unable to witness the signature, the participant’s parent / guardian must sign the form in the presence of a public notary.
THESE FORMS MUST BE COMPLETED, PLACED IN ALPHABETICAL ORDER BY LAST NAME, AND TURNED IN AT REGISTRATION.
IF A PARTICIPANT IS MISSING A WAIVER AT REGISTRATION, THAT PARTICIPANT WILL NOT BE ALLOWED ENTRY TO CAMP UNTIL IT IS COMPLETED.

The Power of Prayer
We believe in the power of prayer at M28 Camps. From start to finish, and throughout the year, your camp experience and group will be covered in prayer by our staff, team, and prayer volunteers.
We want to encourage you to submit specific prayer request for your group, your students, volunteers, churches, and even personal request for you. Please click the link below or email our team (prayer@m28camps.com) to submit prayer requests.
Promotional Material
To help serve you with registration, signups, and promoting your trip to camp this summer, we have included some graphics for you to use in your church. If you see a graphic posted on our socials, you are more than welcome to grab those for your use as well. (Please note that all material is copyrighted and is licensed to you for use in your local church setting only.)
